They say bad things come in a package. If it happened, the series of events will follow.

3 days back, i was asked to approach to countercheck an infusion drip with a junior whom has yet to be a confirmed staff.
Correct addictives added - checked
Correct pint - checked
Name / Identity - checked

but somehow the name and ID was not visible, it was over-write by some words.

Some kind soul discovered it and somehow NM saw it. Asking who put it up..

It was me who counter-checked it. We even bring the sticky label to patient's bed-side and check can!!

The next moment i knew, i was called up into nm's room. Got reprimanded by not guiding my junior well, not adhereing to the IPSG and patient's safetly, did not alarm the pharmacy to change the sticky label and went ahead to hook up the infusion drip.

Got a counselling form, the biggest blow of all


Next.. on night shift... K replacement for patient.. supposed to run 2 hours, but it completed within 1 hour. NM discovered it and tried searching me hi-lo to clarify. (I was on my way home from night shift, phone in silent mode) Finally i saw her message and replied my call. Clarified everything. Got reprimanded again....

"next time i want the rate setting to have counterchecker too. Please counter check for one another. U claimed u set it as 40mph, but no witness.. and the fact the infusion drip completed in 1 hour"

As i listened, my heart just crack a bit. Maybe really it's me who set it wrongly.

Huang Qian Wen: You better reflect all the silly mistakes you've made..

Need a break. Will be going for my AL - Class - next week..... Just let me get away from this environment.

The fact that i got a counselling form makes me feel bad.
